非计划性拔管（UEX）的定义非常清晰的标明发生非计划性拔管自由两种可能：一是患者自定拔除。二是医护操作不当。非计划性拔管是指患者治疗需要而留置在患者体内的各种导管，未经医护人员同意，患者将插管自行拔除，或其他原因（包括医护人员操作不当）造成的插管脱落，又称意外拔管。非计划性拔管率一直居高不下的原因到底是什么？首先，要准确评估导管的重要性。事实上非计划性拔管后有接近半数患者不需要重新置管。他们只是在不正常途径在不适宜的时候拔除了。其次，关于使用镇静与约束工具的时候，医生与护士必须统一目标。防拔管约束普遍用的防拔管约束手套。护士通常是遵医嘱，可医生认为不用使用镇静与防拔管约束手套。造成精细化护理工作，护士无法完成。医生必须根据当前情况，使用镇静与防拔管约束手套，让护士有据可依。再次，由于目前很多医患矛盾的严重化，有些护理安全甚至在人文关怀之下了，肯定是不行的。医护人员必须对不良事件做好预防，与患者达成一致的目的，该使用防拔管约束手套的一定要让患者明白。所以医护人员首先确认这个导管的重要性。其次要确认患者有无拔管风险。第三就是统一思想，对有风险的人实行镇静和防拔管约束手套。根据病情与实际工作作出判断，通过健康教育、思想辅导等一切方法预防非计划性拔管的意外事件的发生。   Read More>
